High blood pressure risk and playing college football linked

College football players, especially linemen, may develop high blood pressure over the course of their first season, according to a small study in the American Heart Association's journal Circulation.

Researchers documented higher blood pressure levels among 113 first-year college players. Only one player had already been diagnosed with hypertension before the season and 27 percent had a family history of hypertension. At post-season, researchers noted:

47 percent of players were considered pre-hypertensive, while 14 percent had stage 1 hypertension.

While previous studies indicate blood pressure elevation during adolescence and young adulthood can increase heart disease and heart-related death later in life, the unique findings of this study suggest early careful monitoring of young football players and timely treatment could improve their heart health later in life.

"High blood pressure is not a good thing at any point in life, but especially during the first two decades," said the study's senior investigator, Aaron L. Baggish, M.D.

"The findings shouldn't scare players," said Baggish, associate director of the Cardiovascular Performance Program at Massachusetts General Hospital. "The earlier in life we can identify and begin treating it the better, and identifying special at-risk groups, like these players, is essential."

From 2006-11, researchers at Massachusetts General Hospital, Harvard Medical School, and Harvard Department of Athletics tracked blood pressure changes among players on the Harvard University team, before and after their first season. Researchers also examined changes in endurance-trained competitive rowers, but found no corresponding increase in blood pressure, suggesting the phenomenon may be related to periodic episodes of intense exertion such as football, Baggish said.

Overall, blood pressure levels averaged 116/64 millimeters of mercury (mm Hg) - which is normal - before the season, but afterward rose to an average 125/66 mm Hg, which is pre-hypertensive.

Players on the offensive or defensive line who gain weight during the season and have a family history of high blood pressure were most likely to have post-season hypertension.

Researchers also noted structural changes in players' left ventricle, the heart's main pumping chamber, which can be a potential indicator of worsening heart health since it can grow thicker if the chamber is overworked. In this study, left ventricle thickening (left ventricular hypertrophy) was more prevalent among football players than endurance athletes, and it was significantly greater among linemen.

"Importantly, left ventricular hypertrophy among football players was strongly associated with resting blood pressure suggesting that heart remodeling in some athletes may be due to what happens off the playing field," he said.

"Considering the popularity of football in the United States, I believe this knowledge of an association with enhanced prevalence of prehypertension and stage 1 hypertension after one season in some players is extremely important," said American Heart Association spokesperson Ernesto Schiffrin, M.D., Ph.D., who is not affiliated with the study. "However, the study should not be interpreted to mean that playing football causes hypertension. Instead, it suggests increased surveillance particularly in those most susceptible: those with a family history of hypertension or playing on the offensive or defensive line."

Professional football players tend to have higher rates of both hypertension and premature death from heart disease, especially linemen, Baggish noted. He and his colleagues are continuing to monitor players identified as at-risk to gain a better understanding of hypertension and heart disease, if and when it develops, as they age.

A class of drug used to lower blood pressure could potentially slow the rate of cognitive decline in dementia and even boost brain power, according to a study published by BMJ Open.

Researchers from Ireland analyzed the cognitive decline and brain power of 361 patients with an average age of 77. All had been diagnosed with either Alzheimer's disease, vascular dementia, or a mixture of both.

Of these patients, 85 were already using the blood pressure-lowering (antihypertensive) drugs known as ACE inhibitors (angiotensin-converting enzyme inhibitors). This is a commonly used class of antihypertensive drug. Thirty of the patients who were newly prescribed ACE inhibitors during the first six months of treatment were also analyzed for their brain power activity.

Between the years 1999 and 2010, the researchers used the Standardized Mini Mental State Examination (SMMSE) or the Quick Mild Cognitive Impairment (Qmci) to test the cognitive decline of each patient. This was done on two separate occasions, six months apart.

The patients who were taking ACE inhibitors had slower rates of cognitive decline in the study compared with patients who were not taking the drugs.

The study also revealed that in patients who had been newly prescribed the ACE inhibitors over the six-month period, their brain power improved in comparison with both those already taking them and those not taking them at all.

However, the study authors add that this could be due to the newly prescribed patients having better control over their medication regimen, or due to better blood pressure control or improved blood flow to the brain.

The researchers say:

"This [study] supports the growing body of evidence for the use of ACE inhibitors and other [blood pressure-lowering] agents in the management of dementia.

Although the differences were small and of uncertain clinical significance, if sustained over years, the compounding effects may well have significant clinical benefits."

The researchers warn, however, that previous research has indicated that ACE inhibitors may be harmful in some cases, so if future benefit of the drugs in dementia is proven, it may be limited to certain groups of patient.

The study authors call for further research and a controlled trial to confirm their findings. They say:

"If these data can be reproduced in a randomized trial of sufficient length, incorporating appropriate outcome measures, such as an amyloid positron emission tomography (PET), then these agents are likely to have significant benefits in delaying or even preventing dementia."

US researchers have designed new molecules that could be used as a starting point for developing drugs that "switch off" high blood pressure in the body.

The scientists first studied natural molecules that the body uses to control blood pressure, identified the parts that are actively involved in switching off the signaling that leads to hypertension, and then designed "analogs" of the natural compounds to do the same.

The work, which focuses on one natural compound in particular, is reported in the July 17th online issue of Bioorganic & Medicinal Chemistry.

Senior author Daniel O'Connor, a professor at the University of California San Diego (UCSD) School of Medicine, says in a statement:

"Our results suggest that analogs can be designed to match the action of catestatin, which the body uses to regulate blood pressure.

Those designer analogs could ultimately be used for treatment of hypertension or autonomic dysfunction."

Catestatin is a natural hormone that acts as the gatekeeper for the secretion of catecholamines - hormones that are released into the bloodstream during times of physical or emotional stress.

A drug that mimics this action would thus help control the hormones that regulate blood pressure.

In the US, around a third of adults - over 70 million people - have high blood pressure, and according to a 2012 report from the Centers for Disease Control and Prevention (CDC), over half do not have their hypertension under control.

If not treated, high blood pressure damages blood vessels and raises the risk of kidney failure, heart attack and stroke.

Yet despite it being a common and sometimes deadly condition, current treatments for high blood pressure only partially control it, and most have serious side effects.

To regulate blood pressure, catestatin binds to nicotinic acetylcholine receptors found in the nervous system.

In earlier studies, Daniel O'Connor and colleagues had already figured out which parts of catestatin bind to the receptor, rather like mapping how the various ridges on a key help open a lock.

In this latest study they studied these "ridges" and developed a three-dimensional chemical model of the most important binding centers.

They then used this 3D model to search a library of 250,000 compound structures for molecules with a similar "fingerprint," tested them on live cells and uncovered some that successfully lowered hypertension.

In further experiments using live mice with high blood pressure, they found that one compound in particular, TKO-10-18, had the same antihypertensive effect as catestatin.

Lead author Igor Tsigelny, a research scientist with the UCSD's San Diego Supercomputer Center (SDSC), says:

"This approach demonstrates the effectiveness of rational design of novel drug candidates."

The team says with further refinement, the model should lead to a new class of drugs to treat high blood pressure.

A simple smartphone app that makes it easier for GPs to help patients with long terms conditions that require regular monitoring, such as high blood pressure, won "Best in Show" at the first ever BMJ hack day in London which took place on 6/7 July 2013.

As Andy Murray clinched the Wimbledon singles title, hackers Andrea Weir and Alok Matta teamed up with Anne Marie Cunningham, a GP and clinical lecturer at the University of Cardiff, to develop a simple yet effective way for patients manage their high blood pressure based on national guidelines.

The judges were impressed by its "simplicity and cheapness" and the team will now work with BMJ to take their idea forward. For example, how the data would be sent to the GP and how the app could be adapted to other conditions such as diabetes and asthma that also rely on patient self management.

The two-day hack, organised for BMJ by Rewired State, began on Saturday morning with an outline of the four challenges by BMJ staff and 13 "hack teams" were given access to the underlying datasets that drive products such as Best Practice, BMJ learning and Onexamination, as well as collections of journal articles.

The hackers were asked to develop ideas aimed at:

Creating a "zero harm" NHSInnovative applications to create better doctors for future healthcare needsHow to "localise" BMJ's published materials to make them more relevant in different countries How to revolutionise the scholarly publishing process

On Sunday afternoon, each of the teams took part in a "show and tell" to present their solutions to the judges.

The winning team outlined key features of their app, including a video on how and when to take blood pressure readings at home - and "push notifications" to remind patients that they need to take a reading.

The app also prevents patients from adding wrong values or adding more readings too quickly. It would work offline and across different platforms and results could be emailed or exported.

In second place was a reward-based game aimed at making revision for medical students more collaborative and less solitary. It was developed by Jon Hilton, Daniel Inniss, Giuseppe Sollazzo, Kaelan Fouwels, and Kush Depala using BMJ'sOnexamination obstetrics and gynaecology questions.

Hilton, a medical student sitting his exams this week, who describes himself as a "self confessed geek" said the technology could also accommodate a doctor who can join in to advise on answers - and could also address BMJ's localisation challenge by testing the translation skills of UK doctors working overseas.

In third place was an idea to address the challenge set by BMJ on the scholarly publishing process, with an open access button that allows researchers to find an open access version of the paper they are trying to read.

The judges felt that, as well as championing open access publishing models, the tool could also function as a "pester power" resource to persuade librarians to subscribe to a particular journal.
